Each year, the NHS collect information on the health of the nation’s teeth. The children have all brought home a consent form tonight that needs completing and returning to school. Above, I have attached the information about the programme for you to have a look at. Not all children will be selected for the survey, they will take a random sample of children on the day. If you are happy for your children to be included, please complete the consent form and return it to school.
